From Book 3. This is so strange. You can see a line from here which stretches through all of those satellite games WFRP, Warhammer Quest (AHQ), Necromunda, Mordheim, Inquisitor where special heroes have lives beyond the battlefield, with careers (background) and experience (continuity), and roleplayed as singular characters (perhaps even conversational roleplay). And it's right here in the first WH battle rules, including hit location rules! I've been reading a lot about the invention of D&D all being tied up with miniature battles (a little earlier in gaming history) - Especially interesting is the role of heroes who act as multiples of normal troops (which is similar to having 3 life points/lives) It's fascinating to see that gradient of mass battle to skirmish and then dueling, and then possibly adventuring (abstract quests, a series of encounters, settings beyond the field, a proto-narrative). But it also makes me sad that Games Workshop shop staff I've talked to over the years have always dismissed roleplaying or quests in relation to the hobby (a slight concession a few years back to Inquisitor), when clearly battle games and roleplaying were always such close dancing partners on the tabletop.
